As a Terem Patient, you are already aware of the fast, professional, care that Terem provides within its Emergency Medical Care Centers. We would like to bring to your attention within these pages additional services that Terem has to offer.

Patient Online Access to Medical Records

Terem has enabled two ways to check your Terem test results.

  1. Directly through Terem's web site
  2. From a service offer by Life on Key.
Below is information about the two options.

Patient Access to Medical Records via Terem's Web Site

All of your x-rays, lab results and patient charts are viewable online by you, via the "Test Results" button on the left of our web pages.

To access your records and test results, you will need your patient number and visit number - both of which can be found on the top of the copy of your patient chart that you were given when you were discharged. You will also need your teudat zehut number.

Instructions to enter the Terem Patient Record database are explained here.

Patient Access to Personal Medical Record

As a part of an effort to improve patient servies, all your visits, including the current one, are saved in a personal medical record by Life On Key. LifeOnKey TM is a revolutionary Personal Health Management platform that allows you, to control your health information in a completely secure, safe and integrated way via the internet, available 24/7. Click here to login your personal medical record. In order to login please use the user name, ID and password that appear in your relase form. The service is given to you by Terem free of charge.

Physician Access to Medical Records

We hope that you have fully recovered, however, if you are still receiving care from your physician for the reasons that brought you to Terem, please bring to his attention that he too can see your records, as long as you named him as your personal physician and he is registered within the Terem ROfeh AMit (ROAM) Patient Record system. Refer him to the appropriate pages under "Health Care Professionals".

Radiology

If at any time you need x-rays or ultrasounds, these can be done at a number of our branches. Call in advance to see if you need to make an appointment and what paperwork you would need to bring from your health fund.

Newletters

You might find of interest our Health Updates and Newsletters that share information about current issues and illnesses and talk about preventative medical care. Check out the travel information that is currently posted within the Community Services.

Other services

We would like to mention that Terem not only has branches that give urgent medical care, but houses clinics that provide care in the areas of breastfeeding, podiatry, dental, speech therapy, and family medicine. Many of these clinics have arrangements with kupot. Look under the individual branches for more information.
